Medical Record Department

Medical Records is a systematic documentation of a patient’s medical history and care given. It serves as a basis for planning patient care, documenting communication between the health care provider and any other health professionals contributing to the patient care, assisting in protecting the legal interest of the patient and the health care providers responsible for the patients care. In addition, it serves as a document to educate medical students, resident physicians, to provide data for internal hospital auditing and quality assurance and to provide data for medical research

Medical Record contains the patient’s identification information, the patient’s health history and medical examination findings. Medical Record also contains a summary of the patient’s current and previous medications a well as any medical allergies.

Facilities Availaible

A reference room in the inpatient Medical Records Department is provided to doctors, nurses and medical students. On the basis of ICD–Xth Revision, retrieval of diagnostic related groups for research purpose, project work, dissertation and case study presentation.

In service education

  • Seminar on Schedule H1 Drugs – the number of drugs on the list, adverse effects of the drugs, causes for monitor records was organized by Sr Lissy Sebastian, In-charge, Medical Stores. Dr Girish Thunga, Asst Professor of Clinical Pharmacy from KMC was the resource person.
  • Anti –diabetic medicine/devices handling and dispensing practice class was held for our staff by BD and Lupin Group of companies in order to cater better services to our customers.
  • Seminar on Medication Errors was conducted by Fresenius Kabi India.
  • Regular classes were taken for the staff giving information regarding the smooth and effective functioning of the Pharmacy.

Facilities added

  • Additional well planned and spacious Pharmacy was inaugurated in the month of March 2015 in the Utility Building to cater to the increased number of patients.
  • Hospital Formulary was updated and circulated to all the Departments and Wards.

Functions:

  • As we have 3 Pharmacies, purchase is done in the main Pharmacy, which has a wholesale license and the stock is transferred to our OP and IP pharmacies.
  • We supply medicines for various medical camps held by the Institutions
  • Chemotherapeutic medicines and critical care medications are supplied with special concessional rates after negotiation with companies.
  • Yearly contract is done for the fast moving and cost effective drugs through negotiation with standard companies.
